How Tight Should It Sit?

You should be able to slide a finger in comfortably. Tighter and the link presses into your skin and the clasp pinches when you bend your wrist. Looser and it turns to the underside, so you mostly see the clasp. For most wrists 20 cm is the middle size, with 18 cm for a narrower wrist and 22 cm if you deliberately want it looser. If in doubt, go a size up: a thick link is sooner uncomfortable tight than loose.
